Home and Community Care supports and improves the care provided by the family and community but does not replace it. Each person is unique; therefore, all services are delivered in a holistic manner according to the person’s physical, social, spiritual and emotional needs.
Based on needs identified through assessment, Home and Community Care services are delivered to registered NFN members residing on-reserve. Services include:
In order to qualify for HCC services, clients must:| Be formally assessed and determined to have a need for these type of services | |||
| Be diagnosed with a medical condition, chronic illness, disability or recovering from an illness/surgery | |||
| Be living at home rather than a long term care facility (as long as it is safe, affordable, and services are available) |
Referrals may be made by clients, family members, friends, neighbors, physicians or other service providers. These services continue for as long as deemed necessary through medical assessments.
Home and Community Care services include Homemaking Services, Meal Preparation, Personal Support, Nursing Services, Home Maintenance, Transportation, Loan Cupboard for Medical Assistive Devices, Diners Club, Lifeline hook-ups, Referrals to other Service Providers.
